Liaise with/arrange for real estate appraisals for sale or rental of the former home if required.
Liaise with the social worker in the hospital to ensure a smooth transition from hospital to facility.
Liaise with current professionals (solicitors, accountants, financial advisers) to ensure that advice is in line with current estate plan, and that any recommendations are carried out in a tax-effective way.
Liaise with Centrelink/DoHS to ensure that Means Tested Fee/Income Tested Fee assessment has been correctly completed, and if not, follow through with Centrelink/DoHS to ensure income and assets are correctly recorded and care recipient is receiving correct assessment.
